MY Medical Record

We have developed a downloadable medical record that you can use to document your health care status.  We have also developed a group of documents that you can bring to the hospital and give to your surgeon, anesthesiologist, and nurses.  The goal of these documents is to make sure that everyone has accurate information about you so as to prevent a medical error.
